G-ASJJ delivered 6.4.65 crashed shortly after take-off from Milan-Linate 14.1.69.

The aircraft was replaced with G-ASTJ a while later. There were three Captains on the aircraft and shortly after take-off the RR Spey 506 engine bleed ring became stuck and caused an engine surge which meant the engine lost power. All three captains identified the problem on the wrong engine and closed it down…. This action resulted in the crash and a modification was made to all 1-11’s afterwards that introduced a red warning light in the handle of the HP Shout off Valve lever so that when an engine failed for whatever reason a light illuminated to show which one it was. Take the decision away from the crew that was the answer!!!
